An exciting opportunity has arisen for a Clinical Sister / Charge Nurse / Clinical Practitioner to join our Care Navigation Centre. You must be a registered practitioner (nurse / paramedic) with evidence of post-registration practice at Band 5. Experience of multi-disciplinary / multi-agency working is essential. You will need to work flexibly to meet service demands and be able to forge strong links with colleagues from both community and acute services. Good IT skills and a pleasant telephone manner are also required.
Service provision is delivered over a 24/7 period across 7 days on a pro-rata system.
