We are looking for an enthusiastic person to come and work on our Postnatal ward at Queen Mary Maternity Unit, West Middlesex University Hospital.
We want to appoint an experienced, motivated and enthusiastic Transitional Care Support Worker to join our well established Transitional Care team based on our friendly postnatal ward.
Main duties of the job
The role of the Transitional Care Support Worker in Maternity is to provide high standards of care in the assessment, planning, implementing and evaluating of evidence based care to babies requiring transitional care within the maternity unit.
The post holder will demonstrate their ability to deliver clinical care under the supervision of the Registered Midwife/ Nurse and will promote a friendly, supportive and safe atmosphere for parents and families in which they can learn to care for their babies.
The post holder will provide direct care to babies requiring transitional care within the ward setting and support women in building their confidence to care for their babies.
The maternity service is a 24 hour service and staff will be expected to be flexible within the agreed working pattern of the unit.

Job responsibilities

* Ability to take handover of daily duties from the midwife and use own judgement when prioritising care of neonate.
* Demonstrates insight that care is appropriate to the neonates assessed needs, context and culture.
* Ability to participate in the documentation of individualised neonatal care plan developed with the mother.
* Demonstrates ability to recognise symptoms that may require discussion, intervention or referral, ie abnormal vital signs using NEWT, abnormal blood values.
* Has insight into importance of escalating any concerns in a timely manner to the midwife, neonatal team and wider MDT.
* Demonstrates competency in examination of the Term baby, this includes checking the baby's appearance, colour, breathing, behaviour, activity, posture and has passed urine and meconium within first 24 hours of life.
* Demonstrates the ability to provide basic care and hygiene needs of the neonate and provide appropriate parent education during hospital stay.

Education and Qualifications
* GCSE or equivalent Qualification
* NVQ Level 3 in Health care or equivalent experience.
* Nursery Nurse (NNEB) qualification /Assistant Practitioner certificate.
* Excellence in Care Certificate
Experience
* Experience of providing high quality care.
* Ability to work as part of a team and to liaise with colleagues in other disciplines
* Understanding of the role and own professional boundaries
* One years formal maternity care experience in a hospital /birth centre/ transitional care unit.
* Experience of using hospital patient record system
