Main duties of the job
* To use their specialist knowledge and skills to undertake assessment, provide psychosocial support and care-planning of individuals under the care of the HWBT, whilst implementing and evaluating treatment of clients.
* To hold a caseload of clients and work closely with the Substance Misuse lead Nurse and Prescribers.
* To line‑manage a team of psychosocial workers, providing support, development, and clinical supervision.

Job Responsibilities

Clinical Responsibilities
* To work as a member of the Wellbeing team, which provides a range of specialist services to substance users and those with psychological and mental health needs, including harm minimisation approaches, motivational interviewing, relapse prevention, care planning, health education and detoxification support, mindfulness and group work.
* To make continuous assessments of the psychological, emotional, social, and rehabilitative needs of young people and young adults, considering the needs of family, local community and sentence plan, with regular evaluation of care plans.
* To provide case management to a defined caseload of clients and collaborate with the Substance Misuse Clinical Team Leader and the Prescribers.
* To use agreed and established outcome monitoring tools and any other relevant quality measures.
* To manage the transfer of individuals between prisons and into the community.
* To counsel individuals about their substance use and/or harmful behaviours, using motivational interviewing and other evidence-based interventions.
* To ensure full risk assessment and risk management is delivered effectively.
* To understand and implement clinical governance.
* To ensure the highest quality of service is delivered by following local and national quality and performance standards, guidance, procedures, and practice guidelines.
* To ensure that all work is performed in accordance with Trust policies and procedures, including incident reporting.
* To support the wider service with additional interventions as appropriate and with relevant training, e.g. initial clinical assessment, second signatory for controlled drugs, urine testing, dry blood‑spot testing, NDTMS reporting, drug strategy reporting and data for equality/diversity of service users on site requesting support from the service, perform CHAT assessments etc.
* To support the integration of the psychosocial team into the wider Health and Wellbeing team, ensuring attendance at HWBT meetings, multidisciplinary formulation meetings, and HC governance and prison meetings as delegated by HWBT manager.
* Work closely with the consultant psychologist to promote the delivery of secure stairs in Feltham A and to deliver the principles of Secure Stairs as required.
* Coordinate and attend all reviews: 5‑day review, 28‑day review and 13‑week review.
* Ensure monthly data collection for HJIPs, OST and naloxone.
* Monitor notification alert of new arrivals on OST and complete the PNAP process on release to community (PNAP – Probation Notification Actioning Project).

Previous Experience
Essential
* Substantial experience working in a drug and alcohol service.
* Experience of working in and delivering services to injecting drug users.
* Experience of providing line management in a team.

* Experience liaising with both the prison and/or community teams.
Education and Qualifications
Essential
* Degree or qualifications relevant to the related position.
* Evidence of continuing personal development in the care assessment and treatment of substance misuse clients.
* RCGP Part 1 trained.
* RCGP Part 2 trained or applicant who is currently enrolled in RCGP Part 2 course.
Desirable
* Degree/Master's in Drugs and Alcohol Studies or equivalent qualification.
* Completed the Motivational Interviewing Training Module.
* Psychosocial intervention skills.
