Job Overview
We are seeking a Band 4 Therapy Assistant Practitioner with a passion for hospital avoidance & community rehabilitation in people’s own homes.
Key Responsibilities
* Deliver a 2‑hour response as part of the National Urgent Care Response targets, working within a large multidisciplinary team (MDT).
* Provide crisis response work within patients’ own homes and short‑term rehabilitation.
* Carry out non‑complex assessments within scope of practice to meet the needs of patients.
* Deliver therapeutic programmes of care in patients’ homes, including exercise programmes, provision of equipment and enabling activities of personal and domestic daily living tasks.
* Progress rehabilitation programmes set by physiotherapists and occupational therapists while working alone in patients’ homes.
* Work autonomously in the community with support from senior staff and collaborate with occupational therapists, physiotherapists, nurses and medics.
Qualifications & Experience
* Band 4 Therapy Assistant Practitioner status.
* Experience delivering exercise programmes, equipment provision and ADL activities in the community.
* Knowledge of community services and an understanding of community urgent response pathways.
Working Conditions
* Caseload of adult patients requiring community rehabilitation, under supervision of qualified therapists.
* Multi‑site working across the Black Country Provider Collaborative partner Trusts (Dudley Group NHS FT, Royal Wolverhampton NHS Trust, Sandwell & West Birmingham NHS Trust, and Walsall Healthcare NHS Trust).
* Flexibility to work across sites to meet patient and service needs.
