The role will support the PCN to improve through delivery, positive communication and prompt reporting to ensure stakeholders are informed and progress is made towards network objectives. Additionally, this role is responsible for the management of the Primary Care Network according to the scope, budget and business requirements of the network. The core objective of the Digital & Transformation Lead role is to support the delivery of ongoing improvement to administrative and clinical services. The role will require experience in business / service improvement, with expertise in transformational change. A thorough understanding of general practice systems and processes is essential. Main duties of the job • Lead an improvement approach to change including building capability for quality improvement within the PCN and system wide approaches to problem solving. • Improve adoption and/or optimisation of new technology to enhance patient access and experience and increase PCN productivity. • Build relationships and facilitate collaboration between practices and the wider system, to support delivery of care to patients. • Review and improve the PCNs digital maturity. • Support population health management. • Support understanding of type and intensity of support/training needs of the PCN and coordinate this support. • Facilitate clinically led innovation and the effective adoption of improvement initiatives, including integrated working at place-based level to improve access to services for patients. • Develop and manage an overall transformation plan for the PCN, including identifying interdependencies, managing risks, considering potential impacts on the wider organisation. • Work with the PCN executive team and member practices to improve data quality, understand demand, capacity and activity and drive improvement. • Support interoperability between a variety of digital solutions to foster collaborative working and improve the efficiency of processes. Job responsibilities Project Management • Deliver significant projects to support the PCN and lead to improvements to health outcomes for the patient population. • Manage a whole lifecycle of a programme of system change. • Manage the set up and delivery of multiple projects within a programme of work, working with colleagues and subject matter experts to scope, plan, design and deliver those projects. • Review project reports and risks logs, advising project leads on escalation processes and support delivery of projects where there may be significant barriers to delivery. • Interrogate and analyse sometimes highly complex information to support the delivery of clear project reporting and project outcomes. • Write and submit high level project reports for the PCN board as required, showing progress against milestones, status, resource requirements, issues, risk and dependencies, making recommendations where a range of options may exist. • Develop key performance indicators and milestones for specific programmes and projects, managing conflicts and ensuring that interdependencies are managed effectively. • Support organisational change and the uptake of initiatives that support excellence. • Demonstrate flexibility and support new programmes and projects where necessary. Finance Management • Maintaining and increasing sources of income analysing and reporting on threats and opportunities • Understanding of the financial implications of contract changes and legislation • Maximising income • Minimising expenditure • Processing of income and expenditure information • Record-keeping • Cash-flow monitoring and forecasting • Management of bank accounts • Purchase and sales ledger processing Liaising with the practice accountant Liaising with the practice bankers Organising the ARRS claim monthly HR Leadership • Liaising with the Alliance HR team in relation to recruitment, integrating of new roles and staff into the PCN teams and member practices. • Ensuring induction of new PCN staff employed via the Additional Roles Recruitment Scheme (ARRS) is completed and ongoing training and supervision is monitored and completed to required standards. • Ensure monthly reports are submitted to the Alliance HR on training, sickness absence and appraisals. Service Improvements • Responsible for the delivery of robust information systems to ensure that all project documentation is stored and used effectively (dashboards, highlight reports, risks and issues logs). • Develop programme and project management tools, reports and templates, that are transferable between projects, and can be used for the whole project lifetime, from design to delivery. • Be a champion for best practice, learning from experience and from others, supporting the spread of innovation within the PCN. Governance • To create and maintain accurate documentation in relation to finances, standard operating procedures, contracts, contacts and decision-making, which is accessible centrally. Communication, relationship building and presentation skills • To work in partnership with the PCN leadership and management team • To identify and develop relationships with practice staff, private providers, the local primary care team, neighbouring PCNs and other key partners to enable good working relationships and promote and share best practice • To deliver technical information in a non-technical and engaging way to a range of stakeholders Person Specification Qualifications Essential • Educated to degree level in relevant area of academic practice or equivalent working experience at this level. Desirable • Project Management qualification. Experience • Evidence of continual professional development relevant to the role. • Extensive experience of leading change projects. • Extensive experience of managing projects and delivering them within agreed timescales and budgets. • Experience of interpreting national policy and implementing this through programmes of work. • Good understanding of the Digital agenda within the NHS and Social Care / Care sector. • Previous experience of managing budgets for complex programmes of work, including budget setting and monitoring. • Finance management. • Ability to implement and manage change effectively
