An excellent opportunity has arisen for an experienced Adult Social Worker to join the Transforming Care Team in Leicester. This specialist locum role focuses on hospital discharge for individuals with complex Learning Disabilities, Autism, and/or Mental Health needs. Working full time on an ongoing basis, you will play a vital role in supporting safe transitions from hospital into the community, using your expertise in tribunal work, court documentation, and Section 117 cases to deliver meaningful outcomes.

What You Will Do
Facilitate safe and effective hospital discharges for individuals with complex learning disabilities, autism, and mental health needs
Complete tribunal and court documentation accurately, ensuring compliance with legal and statutory requirements
Work collaboratively within a multidisciplinary team to develop tailored discharge and care plans
Advocate for individuals’ rights, ensuring appropriate support during discharge and community transition
Engage with families and carers, offering guidance and support throughout the care process
