Main duties of the job
Aspart of our Long Terms Conditions team, including 3 LTC specialist nurses, you will be responsible for areas such as health promotion,chronic disease management and health prevention.
Youwill ideally be a nurse prescriber with experience within a primary care setting, haveexcellent clinical skills and be competent in pro-active management of patientswith chronic diseases including Diabetes, Asthma, COPD, Hypertension andCardiovascular disease.

Job responsibilities
You will ideally be a nurseprescriber with experience within a primary care setting, have excellentclinical skills and be competent in pro-active management of patients withchronic diseases including Diabetes, Asthma, COPD, Hypertension andCardiovascular disease.
You will be responsible for chronic disease management, health prevention, health promotion, as well as supporting the management team in the reviewing of clinical policy and procedure. You will
* deliver high quality care for patients with long-term conditions.
* have understandingof and responsibility to maximise incomegeneration from Quality Outcomes Framework (QOF) / Quality Improvement Scheme(QIS) or other future schemes
* work as an autonomous practitioner,responsible for the delivery of LTC nursing services, working as part of thepractice multidisciplinary team, delivering care within your scope of practiceto the entitled patient population.

* Hold current NMC registration

Experience
* Experience of working in a primary care environment

Availability
* You will be available to work extended hours clinics which currently include some evenings and Saturdays
