Nurse Case Manager — Brighouse (Yorkshire travel required)

We're looking for a Registered Nurse to join Routes Healthcare as a Nurse Case Manager, overseeing complex care packages and leading clinical excellence across our Yorkshire services.

Your role will involve
* Managing complex care packages clinically and holistically
* Developing and reviewing care plans and risk assessments
* Leading clinical training and competency assessments for your care team
* Attending MDT meetings and engaging with commissioning groups
* Auditing care notes and medication records
What we're looking for
* Registered Nurse with a valid NMC pin
* Experience in high dependency or critical care
* Tracheostomy and ventilation experience (community setting desirable)
* Strong communication, organisation, and IT skills
* Understanding of CQC regulations
* Car owner/driver
