Job Overview

Providing leadership to the Neighbourhood Care Network (NCN) to ensure the development and provision of high-quality integrated health and social care services through Place Based Care, for the people who are registered for general practice care within the Newport West Network boundary area.

* To support the development and implementation of strategic and operational plans for the Division, particularly aligning to Place Based Care.
* To provide leadership and secure engagement from Primary, Community and Secondary Care for neighbourhood service planning.
* Work closely with the Locality Management Team and other officers of the Health Board to ensure delivery of the objectives set out for the Neighbourhood Care Networks in respective strategies.
* To support the delivery of high quality and equitable primary care services across the neighbourhood.
* Collaborate with other NCN leads and the wider Health Board clinical management team to provide clinical leadership, motivation and development through GMS Collaboratives and drive forward service transformation at an individual practice level reflective of population need.
* Work with and support practices in undertaking Quality Improvement projects, drawing upon Health Board resources where appropriate.
* Inform and engage where appropriate with the Clinical Interface Group.


Main duties of the job

* Provide leadership and secure engagement from Primary and Secondary Care for neighbourhood service planning.
* Lead or chair localised NCN meetings, provide holistic development and support across contracted services and professions, and contribute to Health Board wide meetings where appropriate.
* Support the delivery of high quality and equitable core primary care and community services across the neighbourhood.
* Drive improvements and innovation in the delivery of services to ensure continuous improvement by reducing waste, harm and variation and supporting the adoption of locally agreed and evidence-based care pathways.
* Assist in assuring appropriate clinical governance arrangements across the NCN.
* Support the Primary Care Clinical Directors and Divisional Nurse in developing a culture where Patient Safety and Quality have the highest priority.
* Drive clinical quality improvements across all aspects of primary and community care in the network.


Essential criteria

* A strong commitment to openness, honesty, inclusiveness and high standards in leadership.
* Professional qualification and current registration or minimum five years recent experience in a senior managerial role in Primary Care, and evidence of continuing professional development.
* Demonstrated evidence of sound financial and staff management.
* Proven record of leading change aimed at raising performance in healthcare.
* A strategic thinker with highly developed communication and negotiation skills.
* A proven track record of commanding respect amongst colleagues, of problem solving and of achieving service improvement and change.
* Strong networker and relationship builder.
* Political astuteness, understanding what can and cannot be delivered at senior level.
* Excellent collaborative working skills and ability to build strong relationships with clinical leaders and professional staff groups, and a proven ability to influence and change those groups.
* Effective and strategic influencing skills.
* Sound knowledge base of the NHS, its current issues, values and principles of timely local access for the population.


Desirable criteria

* Formal qualification in healthcare management.
* Actively involved in a service provided within the NCNs.
* Clinical management experience.
* Demonstrable experience in effective peer group leadership.
* Experience of partnership working, evidence of playing a major role in an organisational change programme.
