The ideal candidate will have previous experience of working in a GP practice setting. You will be caring and compassionate, and will share our commitment to delivering high quality clinical care within a true learning and development environment. You will ideally be experienced in delivering chronic disease management clinics and have undertaken additional training in the management of asthma, COPD and/or diabetes along with wound care management. However, we would be willing to support training of an individual who is keen to develop and there will be a period of overlap with our practice nurse, to provide mentoring and training if required.

JOB SUMMARY
Undertake a range of nursing assessments and provide appropriatecare / treatment in conjunction with GPs according to practice policy, protocolsetc. Strive to improve standards of care and be involved in identifyingpractice population needs. Chronic Disease management is an essential part ofthe role. Knowledge of the GMS contract, e.g. The Quality and OutcomesFramework, LES/DES is a vital element of the post.
DUTIES AND RESPONSIBILITIES
Professional Responsibilities
Assist GP's to help deliver effective implementation ofmanagement of pts with chronic diseases as detailed by the Quality and Outcomesframework.
Support the implementation of evidence-based guidelines inconjunction with the practice nursing team.
Encourage the development of integrated services withinPrimary Care to include integrated practice based teams and multidisciplinaryworking.
Promote your own continuing professional development.
Recognise the opportunities for working in partnership withthe Primary Health care team.
Participate in an annual appraisal.
Clinical: The following skills are desirable but training willbe given if required.
Support the practice in achieving quality indicators andtargets of the GMS contract, PCN DES and any relevant National ServiceFrameworks.
Experience in Chronic Disease Management or willingness toundertake training.
Undertake opportunistic screening when clinically required.
Perform electrocardiography
Undertake simple and complex wound care/ management dressingtechniques, including leg ulcer management, Dopplers and compressionbandaging.
Perform cervical screening and promote breast awareness.
Administer adult and paediatric injections under anindividualised prescription or Patient Group Direction, ensuring safe storage,rotation and disposal of vaccines and drugs.
Provide advice on lifestyle issues, assist and supportpatients in changing their unhealthy behaviours and refer appropriately.
Deliver travel health care, including vaccinations and advicefor patients - following appropriate training if required.
Recognise the signs of child abuse, family violence,vulnerable adults, substance abuse, addictive behaviours, suicidal tendenciesand be aware of local, referral procedures.
Ensure that current evidence based approaches to patients arestandard practice
Apply infection control measures when collecting and handlingof laboratory specimens, disposing of waste materials, dealing with blood andbody fluids spillages and report and treat sharps injuries.
Undertake first aid and management of emergencies, should theypresent at the practice, eg: burns, haemorrhage, hypo/hyperglycaemic emergenciesand asthma attacks.
Undertake annual training and demonstrates competency inanaphylaxis and resuscitation techniques
Research, Audit and Quality
Abide by the legal requirements and statutory rules relatingto practice and record and maintain periodic professional registration with RCN& NMC
Contribute to the development of a practice audit strategy andaction plan for Primary Care.
Contribute to the provision of equitable, high qualityservices for patients in the locality.
Help support the integration across primary, secondary andcommunity services.
Maintain an interest in national research initiatives anddisseminate appropriate information to health professionals if asked to do so.
Ensure all care is evidence based using up to date validatedliterature.
Contribute to research and development programmes whererequired to improve clinical practice. Education and Training
Provide professional advice /support to health careprofessionals within the Primary health care team
Following identified training needs, participate in trainingand facilitation of other practice nurses & health care assistants.
Provide mentorship and guidance for health care assistants
Participate in partnership programmes of awareness raising,detection and prevention of chronic diseases.
Seek out new knowledge of nursing and health by reading,enquiring and partaking in continuous education programme.
Take responsibility for personal development and education.
Actively participate in multidisciplinary education andtraining.
Monitor and maintain consistency of professional standards andtake appropriate action if standards are not met.
Maintain professional development by attending relevantcourses and study days to keep abreast of current knowledge and developments.
Maintain a level of expertise in specific chronic diseasemanagement (when appropriate) and other pertinent areas of health care, andseek to develop new and improved skills.
Advise members of the public on the concepts of healthpromotion, education, prevention and protection.

Person Specification
Qualifications
* Registered nurse. Minimum 2 years post registration. Willingness to learn and acquire new skills. Evidence of appropriate knowledge base. Interest and basic knowledge in long-term conditions.
* Computer skills including use of practice systems such as EMIS. Post-registration study in topics allied to general practice in particular the management of COPD, asthma and diabetes. Basic/advanced certificate of life support. Undertaken mentorship training.
Experience
* Willingness to learn and acquire new skills. Insight and understanding of current issues in nursing and the NHS. Demonstrate an understanding of audit.
* General Practice experience Awareness of the QOF guidelines. Understanding of health and safety issues.
Qualifications
* Willingness to accept additional responsibilities, Good interpersonal skills, both verbal and written Evidence of problem-solving and decision making skills. Good team player. Ability to be reflective, accept criticism and act constructively. Ability to work core hours, Flexibility for cover and working within Network Standard Hours
