Community Mental Health Practitioner / Nurse

The OPMH East DevonCommunity Team, based at St Johns Road in Exmouth, is seeking a committed andcompassionate Community Mental Health Practitioner / Nurse to deliver highquality mental health services across the East Devon area.

About the Role

As a key member of our multi-disciplinary team, you willmanage a defined caseload, providing continuous assessment and personalisedtreatment to individuals with a range of mental health presentations. Thisrewarding role is central to supporting peoples recovery and wellbeing.

* Key working: Design and co-ordinate bespoketreatment packages tailored to individual needs.
* Caseload management: Take responsibility forassessment, planning, implementation, and evaluation of care.
* Liaison and partnership: Work closely withstatutory and non-statutory agencies to ensure holistic support for serviceusers and their carers.
* Team collaboration: Contribute effectively as anintegral part of our multi-disciplinary team, sharing expertise and supportingjoint working.

Main duties of the job

* Contribute to the teams engagement including liaising with a wide range of other professionals and agencies.
* Impart complex and sensitive information to
* Patients and families/carers re assessment, care planning, treatment and review
* Multi-disciplinary teams to ensure the provision of consistent well co-ordinated care
* Primary care teams regarding client care
* External agencies also involved in the provision of care e.g.: voluntary and private sector, police, social services, housing, employment etc
* Service user/carer groups
* Contribute to the safe and effective day to day operations of the service including the handling of clinical enquires.
* Provide and receive information some of which may be contentious, sensitive and complex to and from individuals and their families/carers and actively seek support as appropriate.
* Provide a high standards of written and verbal communication that is clear and in line with professional documentation standards
* Participate in clinical meetings, reviews, professional meetings, and clinical risk meetings.
* Communicate information in a way that makes it relevant and understandable for service users and carers, working in line with practice standards and operational policies.
* Facilitate fair access to social care services and promote social inclusion regardless of age, ethnicity, gender, sexual orientation or disability
* Actively contribute to multi-disciplinary team working.

Person Specification


Skills and abilities

* Excellent communication skills including the ability to engage with clients experiencing a wide range of mental health presentations.
* Ability to communicate with a wide range of professionals from various settings.
* Able to produce well written reports and Computer literacy.
* Able to recognise potential difficulties and participate in finding solutions with the ability to assess risk.
* Able to prioritise work to reflect changing needs.
* Able to work independently and as part of a team.
* Ability to manage own time and the ability to work flexibly.
* Ability to be mobile across a geographical area in a timely manner and in accordance with service need.
* Good clinical assessment skills.
* Ability to manage change in a positive manner.
* Able to participate and present information in a group setting


Experience

* An understanding of the NSF and Mental Health Act/ Mental Health Capacity Act requirements.
* An understanding of CPA Process
* Understanding of recovery principles in Mental Health
* Understanding of Health and Social Care Agenda for Older Adults
* Understanding of Organic Presentations in Older Adults
* Managing a caseload of clients with a range of mental health presentation.
* Experience of working with older adults with mental health problems.
* Working within a community setting in either the statutory or voluntary sector.
* Providing clinical supervision.
* Experience of working in a multi-disciplinary and multi-agency environment.


Qualifications

* An appropriate professional Mental Health qualification and registration e.g. RMN, OT or Social Worker
* Training to level 2 in Safeguarding Adults, children and young people: roles and competencies for Health Care staff
