About the Role:
Looking for an experienced and dedicated Social Worker to join an Adults Locality Team based in Gloucester. This role involves working with adults with learning disabilities, their families, and carers, to ensure the delivery of high-quality, person-centred social care services. The successful candidate will work collaboratively with a multidisciplinary team to assess needs, plan and deliver support, and safeguard vulnerable adults while promoting independence and well-being.

Key Responsibilities:
* Assessments and Care Planning: Undertake person-centred assessments of adults with learning disabilities, including complex cases, and develop tailored care and support plans to meet identified needs.
* Safeguarding: Proactively identify and manage safeguarding concerns in line with the Care Act 2014, ensuring adults are protected from harm.
* Advocacy and Support: Provide advice, advocacy, and support to service users and their families, empowering them to make informed decisions about their care.
* Collaboration: Work collaboratively with health professionals, service providers, and other stakeholders to deliver integrated support services.
* Legislation Compliance: Ensure compliance with relevant legislation and guidance, including the Mental Capacity Act 2005 and Deprivation of Liberty Safeguards (DoLS).
* Case Management: Manage a caseload effectively, maintaining accurate records and meeting statutory deadlines.
* Service Improvement: Participate in service development initiatives and contribute to improving outcomes for adults with learning disabilities.
Requirements of the Social Worker:
* Qualification: Degree or Diploma in Social Work and registration with Social Work England.
* Experience: Previous experience in adult social care or similar settings. Demonstrable experience working within adult social care, particularly with individuals with learning disabilities. Proven ability to manage complex caseloads and make informed decisions under pressure.
* Knowledge: Strong understanding of UK legislation, including the Care Act 2014, the Mental Capacity Act 2005, and safeguarding procedures.
* Skills: Excellent communication, assessment, and decision-making skills. Ability to work autonomously and as part of a multidisciplinary team.
* Other: Willingness to travel for home visits and meetings, where required. A full UK driving licence and access to a vehicle are desirable.
