Join a dynamic and growing home health care agency in Chester on a contract assignment. Local and travel candidates will be considered. We are currently seeking skilled and compassionate Registered Nurses (RNs) to fill multiple contract positions. OASIS proficiency is REQUIRED.
Key Responsibilities:
* Deliver high-quality nursing care in the home setting, following established care plans.
* Conduct initial patient assessments and develop customized care plans in coordination with healthcare teams.
* Administer medications, provide wound care, and manage IV therapy as needed.
* Educate patients and families on disease processes, treatment options, and home care best practices.
* Maintain accurate and timely documentation of patient care in the electronic medical records (EMR) system.
* Monitor patient progress and adjust care plans as necessary to achieve optimal outcomes.
* Communicate effectively with physicians, therapists, and other care providers to ensure comprehensive care.
* Adhere to all safety protocols and guidelines in delivering home health services.
Qualifications:
* Current and unrestricted RN license
* Previous geriatric home health experience
* Strong OASIS proficiency
* Experience with HCHB a big plus
* Strong clinical assessment and care planning skills.
* Familiarity with EMR systems and documentation standards.
* Ability to work autonomously and adapt to changing patient needs.
* Excellent organizational and time management abilities.
* Reliable transportation and a valid driver’s license.
