Job Overview

University Hospital Southampton NHS Foundation Trust is delighted to offer a fantastic opportunity to work with us. Please see below for detailed job description of the role.

Main duties of the job

The recruitment team at the University Hospital Southampton NHS Foundation Trust (UHS) are delighted to be able to offer a fantastic opportunity for a customer focused, friendly and enthusiastic individual, to join our expanding service. The Myeloma service is looking for a motivated and organised individual to take on the new role of Patient Pathway Navigator. The role will involve acting as a centralised point of contact for patients, working alongside the clinical nurse specialist (CNS) team escalating patients’ concerns as appropriate. You will also be pivotal in coordinating the care of patients through their hospital care journey, providing the patient with the relevant information and coordinating required investigations.

Working for our organisation

As one of the largest acute teaching Trusts in England, we offer learning and development opportunities to help you achieve the career you aspire to. All non-clinical roles may involve a mixture of on site and remote working. Specific details and flexible working options can be discussed as part of the interview process. UHS employees are able to access a range of NHS discounts, are entitled to a minimum of 35 days paid holiday (pro rata), and we offer a generous pension scheme. Southampton is an attractive place to live and work situated on the south coast, with an international airport and direct trains to London. The New Forest National Park and beaches of the Jurassic coast are also right on our doorstep. The city offers living costs 20% lower than London and 14 schools rated outstanding by Ofsted.

Detailed Job Description And Main Responsibilities

Your interpersonal skills will enable you to be able to quickly forge good working relationships within the hospital and make you someone our patients can trust to effectively co-ordinate their care.

You will be responsible for identifying any blockages or concerns in patient pathways, and recommending solutions and changes to current processes to improve service delivery and compliance.

Work closely with consultants, specialist nurses, clerical and other staff to deliver a co-ordinated, consistent management of patient cancer pathways. Escalate any clinical concerns to the appropriate clinician i.e., CNS/Medical team.

This role is ideally suited for those candidates who are already working at a Band 3 level or above, and looking to develop themselves within the NHS, whilst actively working in and supporting multiple teams around them.

The successful candidate will have a good grounding in medical terminology and experience of working in a pathway coordination/navigation role or similar.
