Secondary Breast Team Support Worker
The Secondary Breast Team Support Worker will be based within each of the Nursing Teams' establishments.
This role provides support to the specialist nursing team and to patients accessing this service.
The position is primarily non-facing patient but may involve occasional face-to-face interactions. Most duties involve providing information and support via email or telephone, signposting to appropriate services for patients and their families/carers. The role also includes organising and assisting the clinical nursing teams to deliver the Cancer Nursing service efficiently, ensuring patient queries and care are addressed promptly. Responsibilities include processing queries and referrals, guiding clinical and patient queries, and supporting pathway efficiencies for patients with Gynaecology and secondary Breast Cancers.
Key responsibilities include:
1. Providing general, specific, and specialist information to patients and clinical teams regarding Secondary Breast cancer and related services.
2. Ensuring patients are signposted appropriately during and after treatment.
3. Offering support for individuals with non-complex care needs.
4. Engaging with community services as appropriate.
5. Recording patient information on the Trust's systems.
6. Assisting in the day-to-day operations of the Secondary Breast Nursing service, including handling phone calls, referrals, emails, and patient queries.
