Overview

The Mental Health Urgent Care Department is a facility where adults, aged 18 years and above, who are experiencing a mental health crisis can receive an urgent biopsychosocial assessment with immediate care planning. It is a purpose built department based at the Basildon Mental Health Unit on the Basildon University Hospital site.

The post holder will work as a Band 6 Senior Practitioner with the urgent care team providing high quality, evidence-based, holistic assessments to patients attending the service. Assessments must be completed in line with your professional code and Trust policies and clinical guidelines. The service offers full emergency mental health assessments and onward care planning with a diverse and highly skilled mental health workforce.

The role requires working collaboratively to support all aspects of emergency mental health care planning from GP referrals, ambulance conveyance to Mental Health Act Assessments, whilst providing 24/7 access for walk-ins. You will assess, plan, implement and evaluate care based upon individual service user needs, under the guidance of the Clinical Lead as per NICE guidance. You will provide delegated accountability for Band 3 Health Care Assistants, ensuring clinical competence and guidance during shifts. You may deputise for day-to-day co-ordination of the shift within the Urgent Care Department when required.

You will work with key partner organisations, service users, carers, clinicians and other practitioners within the multi-disciplinary team to deliver a 7-day-a-week, 24-hours-a-day service for 365 days a year, where shifts are required. You may be asked to undertake other duties commensurate with the band as required by your manager.

Responsibilities

* Provide comprehensive assessment, care and safety planning for patients attending the Urgent Care Department to ensure a purposeful care pathway and promote recovery.
* Ensure accurate and timely clinical documentation in line with professional Code and Trust policy.
* Develop and maintain specialist clinical skills in the assessment and treatment of people with mental health needs.
* Receive, triage and assess patients, ensuring appropriate nursing triage, assessment, care and support.
* Offer advice on management of patients with mental health issues to other health care professionals, advocating for best practice in line with Trust and national guidance.
* Assess, investigate, treat and discharge or refer patients with a diverse range of minor injuries/illnesses within Urgent Care Department guidelines.
* Feedback outcomes of assessments to clinical leads and implement management plans; ensure appropriate referrals for further assessment.
* Implement effective joint care planning for people who repeatedly attend throughout the urgent care pathway.
* Arrange admission to mental health services in conjunction with Urgent Care Department psychiatrist.
* Support the introduction and delivery of short-term psychological interventions for a specific cohort of individuals.
* Liaise with local system staff and maintain accurate patient records to provide assessment reports to GPs and other agencies.
* Provide advice and monitor use of Mental Health/Capacity Act (1983/2005) and other relevant legislation; provide clinical advice, education and staff support.
* Develop networks and communication channels with services; participate in achieving clinical outcomes and accreditations for the service.


Managerial

* Support the Clinical Lead in ensuring effective care provision and service development of the Urgent Care Team.
* In the absence of the Clinical Lead, ensure referrals, assessment and workload allocation occur timely and flexibly, considering patient and carer needs.
* Prioritise workload within agreed timeframes and participate in team meetings; contribute to new care models using QI methodology.
* Participate in multi-disciplinary and multi-agency decision-making; undertake duties to meet patient quality, safety and performance outcomes.
* Promote understanding of urgent mental health care; support induction of new staff and supervise students as required; provide specialist advice and supervision in line with Trust policies.
* Support the introduction of outcome measures to assess patient experience and contribute to team education and development.


Communication And Working Relationships

* Establish and maintain professional working relationships with allied professionals; ensure effective communication with relatives, carers and appropriate agencies.
* Ensure confidential communication between medical, nursing, social care and health care agencies; maintain documentation of risk to ensure safety.
* Support change within the team and participate in learning and teaching to demonstrate clinical skills.


Responsibilities For Patient/Client Care

* Work flexibly to meet patient needs; develop and maintain individualized care with proactive triage, assessment, planning, implementation and evaluation of interventions.
* Ensure risk assessment and management is integrated into everyday practice; provide information to patients and carers about services.
* Contribute to department patient review procedures to ensure timely clinical decisions; utilize research findings to inform care programs and emphasize evidence-based practice.

Qualifications

* Band 6 Senior Practitioner with experience in urgent mental health care and adherence to NICE guidelines and professional codes.
* Ability to provide supervision and guidance to Band 3 staff and coordinate shifts as required.
