Thepost has become vacant as the existing post holder is leaving to pursue a newventure outside General Practice. We have a long tradition of employing our ownPractice Clinical Pharmacist, being one of the first in Bristol to do so. Wefully recognise the value and important role this brings to the wider team. Thepharmacy teams work is well established into practice pathways and workingarrangements.
Therole of the Senior Clinical Practice Pharmacist is well embedded as a seniorclinical role within the wider practice clinical team. Leading our existing Pharmacyteam, the role of the Senior Clinical Practice Pharmacist is pivotal toimproving the quality of care and ensuring patient safety.
Thesuccessful candidate will have a minimum of five years post graduateexperience, be an independent prescriber with primary care prescribingexperience, and will have completed the Primary Care Pharmacy EducationPathway.
Thepost could be worked as full time over five days, or part time - minimum of22.5 hours over three days.
Competitivesalary, negotiable depending on experience and skillset. 5.6 weeks pro rataannual leave per year with one week study leave.
Leadership of the Practice Pharmacy Team- ensuring the team delivers a comprehensive high-quality service to the patients and staff of the practice
Patient facing roles seeing patients and managing medication related problems and queries. Medication reviews at point of care transfer, especially for older frail, care homes and polypharmacy
Staff facing roles - providing support to general practice staff in relation to repeat prescriptions and medication queries
Quality, governance and improvement - working with the prescribing and wider practice team on safety and medicines optimisation. Leading on initiatives, medicines audit work and local projects and schemes of work
Theteam manage a complex and varied program of working across all aspects ofmedicines management. This includes day to day and more developmental andstrategic work. The post holder would lead this work along with qualityimprovement, audit and development of services. We work closely with the ICBClinical Pharmacist.
Regular clinical supervision by a GP partner with opportunities togrow and develop professionally.

Job responsibilities
The post holder is an experienced clinical pharmacist with previous experience of working in Primary Care, who acting within their professional boundaries and as part of the wider clinical team, will develop and manage medicine management services within the practice.
The post holder will lead the multi-disciplinary pharmacy team, to meet practice service requirements for medicines management.
The postholder will manage systems and be part of the workforce relating to repeat prescribing, authorisations and reauthorisation, acute prescription request, management of medicines on transfer of care and systems for safer prescribing.
The post holder will lead processes and perform medication review of patients with polypharmacy especially for older people, people in residential care homes and those with multiple co-morbidities.
The post holder will provide leadership on medicines optimisation, leading on local ICB projects and schemes of work, quality improvement and clinical audit.
