Community Charge Nurse

Due to internal promotion Andover and Winchester ICTs are looking to recruit full time Community Charge Nurses. The Integrated Community Team model aspires to provide a responsive, safe effective caring service that is well led. The team is commissioned to reflect the needs of the local community. Service aims include providing a local point of access, proactive case management and adoption of a co‑produced approach with patients and service users. The Integrated Community Team, whilst operating from a local hub, delivers a joined‑up approach across a specific locality, to deliver excellence in care and engages with all practices and wider stakeholders in the locality.

The primary role of the Community Charge Nurse is to provide strong holistic leadership to the team and support the Team Matron (Band 7). In conjunction with a range of clinical colleagues, will lead and facilitate a patient co‑ordinated case management approach for people who are most vulnerable to, and at high risk of repeat admissions to hospital. Will participate in the holistic assessment, implementation, and evaluation of programmes of care for patients in the home, without direct supervision. This includes those with chronic disease, terminal illness who wish to die at home and complex care needs. Will work across the caseload and the single point of access, utilising their clinical triage skills to identify the needs of patients, liaising with the correct services to ensure optimum care is delivered.


Main duties of the job

Act at all times with Compassion, to be committed to delivering high quality Care, use effective Communication, be Competent and have Courage when needed. Monitor the quality‑of‑care planning, implementation and evaluation. Inform the Community Matron (Band 7) of any concerns with patients, relatives, visitors or staff that may compromise patient care. Communicate effectively with colleagues, patients and carers so that information is shared in order to meet patients needs. Act as a role model in the promotion of person‑centred practice, and challenge practice which is not person centred. Ensure that discharge and transfer planning for patients is done in a proactive and interdisciplinary manner. Assess and maintain the skills and competencies of the team. Support students in their learning and development.

Ensure new staff receive a proper induction so that they understand acceptable practice and standards. Develop own professional skills and ensure these are shared with the team and wider division. Provide management responsibility for the team in partnership with the Community Team Matron. This may include annual leave management, sickness/ absence management; performance management, training and development, appraisals, incident and complaints management. Deputise for the Integrated Community Team Matron as required. Building and maintaining effective relationships with all stakeholders. Ensure effective management of own and team caseload.
