Care Coordinator
Our client is a campaigning organisation and a multi‑project service provider based in West London, supporting disabled people, those experiencing other disadvantages and their families to live healthier, happier, and inclusive lives by ensuring their needs, rights and aspirations are met.
Key Responsibilities
* Provide exceptional customer service, accurate information, and support tailored to meet staff and client needs.
* Ensure policies and procedures comply with CQC regulations and work to achieve the best care standards.
* Produce staff rotas on a monthly basis and manage staff cover during absences.
* Participate in the out‑of‑hours emergency on‑call rota.
* Conduct and maintain client risk assessments and care plans.
Qualifications
* Qualification in Health & Social Care – Minimum of NVQ level 2 or 3.
* Excellent communication and writing skills.
* Caring, compassionate and understanding towards people in need of care and support.
* Self‑motivated and keen to learn.
