This rarely offered and exciting role has now become available. As the Cabrini Palliative Care Liaison Nurse, you will bring advanced clinical expertise and compassionate leadership to the coordination of high-quality palliative care across Cabrini Health. This senior role is ideal for an experienced nurse who thrives on complex clinical decision-making and collaboration.
Working closely with Medical Specialists, advanced trainees and multidisciplinary teams, you will provide expert assessment, consultation and coordination of care for patients with complex palliative needs. You will be a key clinical resource for symptom management and end-of-life care, supporting patients, families and clinicians to achieve care aligned with patient goals and preferences.
In addition to clinical leadership, you will contribute to service development, staff education and quality improvement initiatives, influencing best practice and strengthening palliative care delivery across the organisation. Key Responsibilities
7. Coordinate and respond to palliative care referrals in a timely manner
8. Consult with patients, families/carers and the multidisciplinary team regarding care planning and goals of care
9. Collaborate with Palliative Care Consultants and Registrars to support patient review
10. Liaise with inpatient wards, community services and external providers to support continuity of care and complex discharge planning
11. Provide education and clinical support to staff using evidence-based resources
12. Facilitate referrals to specialist and community palliative care services
13. Support the Supportive Care Clinic and contribute to service development initiatives
14. Participate in committees, accreditation activities and quality improvement
About You
You are an experienced and compassionate Registered Nurse with strong expertise in palliative care and the ability to work both independently and collaboratively.
You will have:
15. Current AHPRA registration as a Registered Nurse
16. Minimum 10 years’ nursing experience
17. Significant palliative care and/or community palliative care experience
18. Knowledge of PCOC and evidence-based palliative care resources
19. Understanding of relevant legislation, accreditation standards and professional competenciest
20. Excellent communication, leadership and stakeholder engagement skills
