Role Description
As a Care Assessor, you will be responsible for managing new care packages, carrying out client assessments, and developing person-centred care plans. This role is a mix of fieldwork and office-based tasks, giving you the opportunity to directly impact people's lives while supporting the wider care team.
Key Responsibilities
* Conduct individual needs assessments for new and prospective clients
* Complete and update risk assessments and person-centred care plans
* Carry out introductory visits when Care & Support Workers meet new clients
* Conduct routine reviews and make care plan adjustments as required
* Support the Care Manager with compliance, audits, and training
* Allocate Care & Support Workers to cover absences (sickness/holiday)
* Monitor client satisfaction and Care & Support Worker performance
* Ensure accurate and up-to-date documentation in both client homes and electronic systems
* Provide cover for the Field Care Supervisor role when required
* Work closely with the Care Manager, Field Care Supervisors, and Care Coordinators
Requirements
* Degree
* Previous experience in the care or healthcare sector
* Care Coordinator experience (desirable)
* Strong communication and organisational skills
* Ability to work independently and within a team
* Full UK driving licence and access to your own vehicle
