In the Older People & Hospital Social Worker role, you will:
* Provide high-quality assessments, support planning, and interventions for older people and adults being discharged from hospital settings
* Work closely with multidisciplinary teams, families, and external agencies to ensure safe and effective care pathways
* Contribute to safeguarding and promoting the wellbeing and independence of service users
* Manage a varied caseload in line with statutory requirements and departmental procedures
To be successful in this role, you will need:
* A recognised Social Work qualification and a minimum of 3 years’ post-qualifying experience
* Registration with Social Care Wales
* Strong IT skills, alongside knowledge of relevant systems and software
* A solid background in hospital social work and working with older people
* Excellent interpersonal, communication, and teamwork skills
This is a full-time role, working hours are Monday to Friday, 37 hours per week. You’ll start this temporary position for an initial 12 weeks, with potential for extension. A competitive hourly rate of £28.01, plus overtime and other benefits, is offered.
The position is based in offices in Colwyn Bay, and there is an expectation that you live locally or within a commutable distance, as the role is located in Conwy County.
