About the role
Location: Great Yarmouth, Norfolk
Hours: Full time, 37.5 per week
Salary: £27,861.26 - £32,002.35, dependent on experience (based on full-time hours, pro rata for part-time)
We are a charity focused on supporting Service Users with complex needs. The post holder will work in a multi-disciplinary team in partnership with Mental Health services and healthcare to engage with people with physical and/or mental health issues and substance use challenges. The role involves holding a small caseload and providing bespoke support and flexible interventions to Complex Needs Service Users.
Responsibilities
* Coordinate and lead a solution-based approach, engaging Service Users in appropriate services
* Carry a caseload of complex clients, developing personalised action plans
* Participate in multi-agency initiatives to reduce risk
* Work closely with drug and alcohol teams to ensure access to treatment for substance misusers and problem drinkers
* Ensure Service Users have access to ongoing healthcare
About you
* Experience in one or more of the following: substance misuse, alcohol misuse, mental health
* Experience working with people who have multiple and complex needs
* Knowledge and experience of multi-agency working and facilitating multi-agency meetings
* Excellent communication, negotiation and advocacy skills
* Ability to build and maintain relationships with a wide range of services
* Ability to work autonomously as well as part of a team
* Good IT literacy, including MS Office applications
* Ability to critically reflect and evaluate interventions and service delivery
