The GMMH rehabilitation service has an exciting secondment opportunity for a dynamic and innovative mental health practitioner to join the Home Engagement and Rehabilitation Team. The service is establishing a strong reputation for providing bespoke, intensive and individualised care to service users living with enduring mental health problems, enabling them to live their lives to their full potential.
Responsibilities
* Play a central role within the MDT to ensure high-quality, recovery-focused care is delivered to community service users.
* Co‑coordinate a small caseload of service users, leading on assessment, collaborative care planning and provision of interventions to encourage self‑management, optimise functioning and promote choice and inclusion.
* Travel across the Greater Manchester footprint – Manchester, Salford, Bolton, Trafford and Wigan – to support service users.
* Support service users to identify their own goals and coordinate timely and appropriate interventions to help them achieve these.
* Respect individuality, values, culture and religious diversity of service users and colleagues, contributing to a service sensitive to these needs.
* Maintain a recovery focus and demonstrate skill and sensitivity in the assessment of vulnerable service users with higher levels of need.
* Organise and supervise recovery workers within the team, respond to crises when necessary, and delegate tasks to colleagues appropriately.
* Assist the leadership team in day‑to‑day operations, organising, leading, teaching, supervising and supporting junior colleagues.
Qualifications & Requirements
* Good understanding of the principles of mental health rehabilitation.
* Awareness of the delivery of community mental health care.
* Ability to work extended hours (8 am – 8 pm, Monday–Sunday) on a rota basis.
* Familiarity with a recovery-focused approach and assessment of vulnerable service users.
