Salters Meadow Centre is looking for a qualified Clinical Pharmacist with a prescribing qualification, ideally with experience working in Primary Care. We are a friendly and welcoming GP Practice in Burntwood, acheiving high QOF results and maintaining a good CQC Standard.
Main duties of the job
We are seeking a motivated pharmacist to join our team. You will be working alongside the GP Partners and clinical team.
You will deliver patient facing clinics, repeat prescription management, acute prescription requests, medication reviews, respond to queries and patient safety alerts. Please see job description for further details.
You will be an independent prescriber.

Salters Meadow Centre provides care for approximately 11500 patients. We have an established team of 4 GP Partners, Advanced Nurse Practitioner, 4 Practice Nurses, a HCA and Phlebotomist. We are also supported by the PCN staff.
We are a training practice and we are an active member of Burntwood Primary Care Network.

Job responsibilities
The post holder is a pharmacist, who acts within theirprofessional boundaries, supporting and working alongside a team of GPs ingeneral practice. In this role they will be supported by the Clinical Team whowill develop, manage and mentor them.
The post holder will work as part of a multi-disciplinary team ina patient facing role. The post holder will take responsibility for areas ofchronic disease management within the practice and undertake clinicalmedication reviews to proactively manage patients with complex polypharmacy.
The post holder will provide primary support to general practicestaff with regards to prescription and medication queries. They will helpsupport the repeat prescriptions system, deal with acute prescription requests,and medicines reconciliation on transfer of care and systems for saferprescribing, providing expertise in clinical medicines advice while addressingboth public and social care needs of patient in the GP practice.
The post holder will provide clinical leadership on medicinesoptimisation and quality improvement and manage some aspects of the quality andoutcomes framework and enhanced services.
The post holder will ensure that the practice integrates withcommunity and hospital pharmacy to help utilise skill mix, improve patientoutcomes, ensure better access to healthcare and help manage workload. The roleis pivotal to improving the quality of care and operational efficiencies sorequires motivation and passion to deliver excellent service within generalpractice.
