We are excited to offer a unique opportunity to join a newly funded project under the Birmingham Fairer Future Fund, aimed at tackling health inequalities through culturally sensitive and collaborative care pathways. This role will be central to the co-design and deliveryof new assessment models, working closely with system partners and community stakeholders to improve awareness and access—especially for under-represented groups.

Essential Criteria:
• Registered healthcare professional with ACP qualification.
• Experience in dementia care or memory services.
• Strong commitment to reducing health inequalities.
• Excellent communication and stakeholder engagement skills.
• Ability to work autonomously and collaboratively in a multi-agency environment.
