The successful candidate will work atAlma Medical Centre, Stockton as part of a dedicated and enthusiastic team,working alongside GP's, Advanced Nurse Practitioner, Practice Nurse, NursingAssociate, HCA's and Social Prescribers.

We are looking for a dynamic ANP withexcellent communication skills to undertake a variety of duties including sameday face to face consultations, telephone triage, path labs, and queries dailyand home visits.

Knowledge of the GMS contract, e.g. The Quality and Outcomes Framework,is a vital element of the post. Additionally, the successful candidate will beencouraged and supported to undertake any further training required to ensureappropriate role development.


Main duties of the job

Providing care and treatment topatients requiring same day appointments for minor illness.

Appointments will be provided througha mix of F2F and telephone appointments.

You will be involved in clinicalmeetings and be part of the process to improve care for patients.

Assist GP's to help deliver effective implementation of management ofpatients with chronic diseases as detailed by the Quality and Outcomesframework.

Undertake male, female and family planning assessments, give advice andrefer patients appropriately.

Job responsibilities

* The Advanced Nurse Practitioner will undertake a range of nursingassessments and provide appropriate care / treatment in conjunction with GPs, othernurse practitioners and practice nurses or independently as appropriate for theneed, according to practice policy, protocols etc. The Nurse Practitioner willstrive to improve standards of care.
* Contribute to the development of policies / protocols / guidelineswithin the practice.
* Support the implementation of evidence-based guidelines in conjunctionwith the practice nursing team.
* Promote the continuing professional development of yourself and othernurse practitioners' nurses, nursing associate & HCAs within thepractice. Working alongside health care assistantsto help develop their skills.
* Provide professional and managerial support to, practice nurse, nursingassociate, health care assistants.
* Provide advice on lifestyle issues, assist and support patients inchanging their unhealthy behaviours and refer appropriately.
* Recognise the signs of child abuse, family violence, vulnerable adults,substance abuse, addictive behaviours, suicidal tendencies and is aware oflocal, referral procedures.
* Ensure that current evidence-based approaches to patient are standardpractice
* Apply infection control measures when collecting and handling oflaboratory specimens, disposing of waste materials, dealing with blood and bodyfluids spillages and report and treat sharps injuries.
* Undertake first aid and management of emergencies, e.g. burns,haemorrhage, hypo/hyperglycaemic emergencies/asthma attacks.
* Undertake annual training and demonstrates competency in anaphylaxis andresuscitation techniques
* Provide professional advice /support to health care professionals withinthe Primary health care team
* Provide mentorship and guidance within the nursing team.
* Participate in partnership programmes of awareness raising, detectionand prevention of chronic diseases.
* Maintain a level of expertise in specific chronic disease management(when appropriate) and other pertinent areas of health care and seek to developnew and improved skills.
* Advise members of the public on the concepts of health promotion,education, prevention and protection
* Take responsibility for personal development and education.
* Actively participate in multidisciplinary education and training.
* Monitor and maintain consistency of professional standards and takeappropriate action if standards are not met.
* Maintain professional development by attending relevant courses andstudy days to keep abreast of current knowledge and developments.
* Abide by the legal requirements and statutory rules relating to practiceand record and maintain periodic professional registration with RCN & NMC.
* Participate in an annual appraisal.
* Contribute to and work effectively within the primary care team
* Assist in clinical audit and the setting and monitoring of standards ofcare.
* You are responsible for limiting your actions to those which you feelcompetent to undertake. If you have any doubts about your competence duringyour duties, you should immediately speak to the Nurse Manager in the firstinstance.

REGISTERED HEALTH PROFESSIONAL

All employees of Alma Medical Centre who are required to register with aprofessional body, NMC, to enable them to practice within their profession, arerequired to comply with their code of conduct and requirements of theirprofessional registration.

SUPERVISION

Where the appropriate professional organisation details a requirement inrelation to supervision, it is the responsibility of the post holder to ensurecompliance with this requirement. If you are in any doubt about the existenceof such a requirement, in the first instance advice should be sought from theNurse Manager.

RISK MANAGEMENT

It is a standard element of the role and responsibility of all staff of AlmaMedical Centre that they fulfil a proactive role towards the management of riskin all of their actions. This entails the risk assessment of all situations,the taking of appropriate actions and reporting of all incidents, near missesand hazards.

RECORDS MANAGEMENT

As an employee of Alma Medical Centre, you are legally responsible forall records that you gather, create or use as part of your work within theOrganisation (including patient health, financial, personal andadministrative), whether paper based or on computer. All such records areconsidered public records, and you have a legal duty of confidence to serviceusers. You should consult the Practice Manager if you have any doubt as to thecorrect management of records with which you work.

HEALTH AND SAFETY REQUIREMENTS

All employees of Alma Medical Centre have a statutory duty of care fortheir own personal safety and that of others who may be affected by their actsor omissions. Employees are required to co-operate with management to enablethe practice to meet its own legal duties and to report any hazardoussituations or defective equipment.

Person Specification


Qualifications

* The ideal candidate will be an experienced and motivated Advanced Nurse Practitioner and have appropriate qualifications with the title of ANP as outlined by the NMC and RCN and have experience in in either General Practice, urgent/acute/out of hours care. Essential qualifications are; Registered first level nurse MSc or equivalent, hold a relevant nursing/health degree plus non-medical prescribing is essential.
