Role Overview
This position is for a Community Support Worker who will be part of a multi‑disciplinary team providing support to people with complex trauma, psychosis, and enduring mental health needs in Lincoln and surrounding areas.
Lincolnshire Partnership NHS Foundation Trust has been selected to transform community mental health services as part of the NHS Long Term Plan.
Responsibilities
* Implement care plans and care packages for service users with severe mental illness or mental health problems within a defined locality.
* Work under the supervision of Community Psychiatric Nurses, Social Workers, and Occupational Therapists, and be managed by the Team Coordinator.
* Collaborate with housing providers, CMHTs, and third‑sector organisations to meet recovery needs of individuals accessing the service.
