About the Role
As part of a multi-disciplinary team, you will be central to the early identification, review and ongoing support of patients with conditions such as diabetes, hypertension, COPD, and heart disease. This is a mobile/ remote role, working closely with practice teams to deliver joined up care and help patients take control of their health.
We are looking for someone who is autonomous, forward-thinking, organised, and confident working independently across different clinical settings.
Key Responsibilities
* Conduct health checks (e.g. BP, BMI, lifestyle assessments)
* Support proactive reviews for patients with long-term conditions
* Perform phlebotomy, ECGs, spirometry, and other clinical duties
* Promote preventative care, health education and self-care strategies
* Maintain accurate, timely and thorough clinical records
* Liaise effectively with GPs, nurses, care coordinators and wider teams
* Travel between 7 practices (mileage reimbursed where applicable)
Person Specification – You Will Be:
* Autonomous and confident, able to work independently across sites
* Organised, efficient, and excellent at time management
* Experienced in managing long-term conditions within a primary care setting
* Skilled at communicating clearly and professionally within a multidisciplinary team
* A natural problem-solver with strong decision-making skills
* Computer literate with experience using clinical systems and apps, including SystemOne and EMIS Web
* Supportive of patients through preventative care and self-management plans
* Forward-thinking, flexible, and adaptable to new models of care
* Ideally from a medical background with relevant clinical qualifications
