The post holder will be an autonomous practitioner, working within codes of practice and managing a varied and complex caseload, providing highly skilled Occupational Therapy interventions for Monmouthshire residents in their own homes. The aim is to facilitate the maintenance and improvement of wellbeing to enable residents to complete activities of daily living through Reablement and other OT interventions.
The post holder will work alongside other Band 6 Occupational Therapists, providing ongoing input and assessment for patients across Monmouthshire, ensuring timely discharge, and leading on tasks such as commissioning and reviewing long-term care and support. The role involves developing expertise within the clinical specialism and practicing occupational therapy safely.
Key Responsibilities:
* Plan and prioritize own patient workload.
* Assess and address occupational performance skills, including mobility, sensory deficits, intrapersonal and interpersonal skills, cognitive skills, self-care, productivity, and leisure.
* Interpret patient conditions and provide interventions in complex cases autonomously.
* Communicate condition-related information to patients and families with empathy and reassurance, supporting decision-making and goal setting.
* Identify and order equipment to help patients maintain independence at home.
* Maintain records and documentation in accordance with standards.
* Participate in case conferences and multidisciplinary teams.
* Adhere to the COT code of conduct and manage risks appropriately.
* Develop and maintain collaborative relationships with colleagues and external agencies.
