Senior Support Worker - Frailty Virtual Ward

We are excited to be able to recruit a Senior Frailty Support Worker to join our existing teams in the delivery of the Royal London Hospital's Frailty service.

The Senior Frailty Support Worker will support with the delivery of care for older adults with frailty under the care of the Tower Hamlets Frailty Virtual Ward. This fixed term post will include weekend working therefore the postholder must be available to work within a flexible rota.

As part of your role, you will be visiting patients in their own home to take blood tests and observations. You will support the Frailty Practitioners in carrying out follow-up reviews and treatments plans under the direction of the team. You will support patients and their families by managing health records, arranging transport and appointments, managing referrals and completing other administrative tasks as required. You will take part in regular MDT meetings where you will take notes and document plans and actions.

This post forms part of the expansion of the Frailty Team within Older Peoples Services at the Royal London Hospital. The team includes doctors, specialist nurses and therapists who care for frail and elderly patients in the Emergency Department, the Acute Admissions Unit and throughout the hospital as well as in their own home


Main duties of the job

We are looking to recruit a Senior Support Worker to join our existing team of Specialist Frailty Practitioners (nurses and therapists) in delivery of the Royal London Hospital's virtual and inpatient frailty services. This is a fixed term post that includes weekend working and the postholder must be available to work within a flexible rota.

The Virtual Frailty Ward supports older patients living in the borough of Tower Hamlets in their own homes. You will be working alongside the Band 7 Frailty Practitioners, the GP's and Consultant Geriatricians. You will take part in regular MDT discussions. You will be supported in your learning and development by an expert team.

Person Specification


Qualifications

* A Level or equivalent standard of education
* Knowledge of a range of IT packages and able to use Microsoft Word and Excel
* Experience of working with the general public


Experience

* Experience of working within an acute care setting
* Experience of working within a multi-disciplinary team
* Experience of working within a community care setting


Skills

* Keep accurate and complete records of activities and communications consistent with legislation, policies and procedures
* Administration skills including maintaining and updating computer records and databases.
* Communicate utilising a variety of strategies to communicate effectively with patients and those involved in their care
* Moving and Handling skills
* Ability to organise and prioritise own delegated workload
