This is an exciting opportunity to join a bespoke community team working with service users with mental health and forensic backgrounds. The post holder will work with individuals under the care of the Specialist Community Forensic Team, providing support in secure hospitals and community settings. You will build therapeutic relationships to enhance discharge pathways and promote independence within a recovery-focused team. You will be part of an intensive multidisciplinary team, contributing to risk management under civil and forensic sections of the Mental Health Act (1983). Our team includes a Consultant Psychiatrist, Clinical Psychologist, Social Workers, Mental Health Nurses, Occupational Therapist, and Senior Recovery Worker.
Key Responsibilities:
1. Develop rapport with service users within appropriate boundaries.
2. Encourage service users to engage with their care plans and access relevant services and activities.
3. Implement care programmes collaboratively with service users and carers under supervision.
4. Monitor signs of relapse by assessing progress, functioning, and mental state, and alert staff as needed.
5. Participate in the evaluation of care provided.
6. Support carer needs as required.
7. Promote health, including physical health needs.
8. Support involvement in social activity groups.
9. Maintain accurate service user records.
