Job Summary

Come and join our great Home Visiting Team in Canterbury! We work in an integrated and collaborative way with Canterbury GP Practices. We are proud of the services we provide and passionate about improving them further. The closing date is 24 June 2026.


Responsibilities

* Work as part of a multi‑disciplinary team within Canterbury Integrated Healthcare, including weekly frailty and/or care home MDT meetings.
* Assess and triage housebound patients, including same‑day triage, and provide definitive treatment (including prescribing medications following policy, patient group directives, NICE and local clinical guidelines) or make necessary referrals to other members of the PCN team.
* Provide direct clinical care to patients using established clinical guidelines for routine and urgent complaints; advise patients on general healthcare and promote self‑management; signpost to PCN social prescribing service and other community or voluntary services where appropriate.
* Liaise with other professionals, families and carers to provide clinical information relating to patient management.
* Support the delivery of Enhanced Health in Care Homes, including admission assessments, supporting care‑home ward rounds and advanced care planning.
* Travel across the Canterbury practices’ catchment area using own transport.


Person Specification

* Essential Experience
o Excellent interpersonal and communication skills.
o Time‑management and ability to prioritise workload.
o Ability to work autonomously and use own initiative.
o Ability to assess and manage risk when working with individuals.
o Experience of working with patients in their own homes or a care setting.
* Desirable Experience
o Knowledge of EMIS Web Clinical System.
o Knowledge of partner products including Docman, Dart and AccuRx.
o Experience of working in primary care.
* Essential Personal Qualities
o Flexible approach to the evolving nature of the role.
o Ability to work collaboratively in a team.
o Drive to provide excellent integrated patient care.
o Ability to work under pressure to meet deadlines.
o Ability to reflect on own performance and consider improvements.
* Essential Requirements
o Access to own transport and ability to travel across the locality.
o Flexibility to work outside of core hours when required.
o Ability to work remotely when required.
o Disclosure Barring Service (DBS) check.
o Requirement to work Monday and Tuesday; other hours can be negotiated to form a fixed rota.
* Essential Qualifications
o Paramedic, Advanced Clinical Practitioner or Enhanced Nurse Qualification.
o Registered with appropriate professional council.
o Commitment to maintain mandatory and role‑specific training.
* Desirable Qualifications
o Experience in long‑term condition management.
o Independent prescriber or working towards an independent prescribing qualification.
o Completed the First Contact Practitioner (FCP) pathway outlined by Health Education England (HEE).


Compensation and Working Pattern

Salary: depending on experience. Contract: Permanent. Working pattern: Full‑time or part‑time.
