Qualified Social Worker Hospital Discharge - Initial Response / Front Door Location: North West Pay rate: £35 per hourWe are looking for experienced and passionate Social Workers to join a dedicated Hospital Team within Initial Response / Front Door. You will ensure that practice is of a good quality and meets the needs of patients, service users and their families. You will supervise the day to day management of those cases for which you are the named worker. Social Workers will intervene with legal and procedural authority to protect vulnerable people at risk of abuse or neglect; to help individuals, families and groups to sustain and develop their capacities for independent living and in complex situations to try to prevent or resolve conflict which may place individuals at risk.Working in accordance with social care legislation and local government requirements, informed by the values and principles of social care; you will be supporting in the delivery of person centred, safe and effective social care services. Using your professional knowledge and experience you’ll make assessments and decisions about the most appropriate interventions.
