Overview
The Saltaire & Windhill Medical Partnership is seeking an enthusiastic and skilled Health Care Assistant to join our clinical team. Ideally you will be qualified and experienced in delivering phlebotomy, long term condition reviews, wound care and assisting with minor surgery and contraceptive procedures. Applications are welcome for both full time and part time hours.
Main duties of the job
ECG recording, phlebotomy, LTC reviews, assisting with diabetes clinics, injections under PSD, vaccine clinics, testing of urine samples as requested by clinicians, diabetic foot checks, NHS Health Checks, asthma reviews (subject to training), wound care, BP checks, chaperoning duties, processing and management of lab samples requested by GPs and Nurses, vaccine and cold chain storage, monitoring and recording, stock control and stocking rooms, assisting with minor surgery and contraceptive services, and administering as necessary.

The Saltaire & Windhill Medical Partnership is a four-site practice based in Saltaire, Shipley, Baildon and Cottingley. We have over 100 staff providing patient services to a patient population of 24,500.
Job responsibilities
Job Summary
To support the practice team in the delivery of nursing services, working as part of the practice multidisciplinary team, delivering care within specific practice guidelines and protocols. The health care assistant will deliver and assist clinical staff in the provision of treatment, preventative care, health promotion and patient education.
Main Responsibilities:
* ECG recording
* Long term condition reviews
* Assisting with Diabetes clinics
* Spirometry subject to training
* Injections under PSD
* Testing of urine samples as requested by clinicians
* Diabetic foot checks
* Asthma reviews subject to training
* NHS Health Checks
* Blood Pressure Checks
* Processing and management of laboratory samples requested by GPs/nurses
* Vaccine/cold chain storage, monitoring and recording
* Surgical equipment and vaccine restocking and stock rotation
* Stock ordering, control and restocking consulting rooms
* Clinical administration as necessary
* Preparing and maintaining environments and equipment before, during and after patient care interventions, including assisting GPs during the performance of minor operations and contraceptive procedures
* Assisting in the assessment and surveillance of patients health and well-being
* Undertaking specific clinical activities for named patients delegated and taught specifically in relation to that individual
* Helping to raise awareness of health and well-being and how it can be promoted
* Assisting with the collection and collation of data on needs related to health and well-being

Training and Development
Stay up to date through attendance at any courses and/or webinars necessary to ensure that professional development requirements are met, demonstrating skills and activities to others undertaking similar work. Subject to a performance review, including taking responsibility for maintaining a record of own personal and/or professional development.
Governance
Support and participate in shared learning across the practice and wider organisation. Manage, review and identify learning from complaints, incidents and near-miss events relating to the practice, team and self. Ensure compliance with policies, procedures and guidelines for self and others, by acting or alerting management if the practice appears to contravene policy, or if there are concerns over any aspect of patient care.
Confidentiality
Maintain confidentiality of information, acting within the terms of the Data Protection Act and Caldicott guidance on patient confidentiality at all times. Maintain an awareness of the Freedom of Information Act. Information relating to patients, carers, colleagues, other healthcare workers or the business of the practice may only be divulged to authorised persons in accordance with the Practice policies and procedures relating to confidentiality and the protection of personal and sensitive data
Health & Safety
Comply with Practice health and safety policies by following agreed safe working procedures. Actively reporting health and safety hazards and infection hazards immediately when recognized. Keeping work and general areas clean and tidy, and using appropriate infection control procedures to keep work areas hygienic and safe from contamination. Correct personal use of Personal Protective Equipment (PPE) and ensuring correct use of PPE by others, advising on appropriate circumstances for use by clinicians, staff and patients. Making effective use of training to update knowledge and skills
Safeguarding
The post-holder is expected to be aware of adult and child safeguarding responsibilities in the context of the post and to undertake necessary training, both at induction and on an ongoing refresher basis as appropriate. They will be familiar with Practice safeguarding policies for children, adults, domestic abuse and Prevent and be confident in raising any concerns through the appropriate channels.

Person Specification
Experience
* Experience of working in a primary care or health care environment.
* Experience of working with the general public.
* Wound care/removal of sutures and staples.
* ECGs, 24 hour BPs, venepuncture, LTC Reviews and Spirometry.
* SystmOne User Skills.
* Competent in the use of Office and Outlook.
* Willing to adapt and develop as the Practice requires.
Qualifications
* Healthcare qualification (Level 3 or 4) or working towards gaining equivalent level.
* Care Certificate.
