Due toincreased demand, we are looking for a Health Care Assistant/Phlebotomist tojoin our friendly Practice family.Oswald Medical Centre is an award-winning Practice serving over 16,000patients. We are based across 3 sites inAccrington, Oswaldtwistle and Blackburn.We pride ourselves on our approach to providing caring services andoffer a range of standard NHS and enhanced services including minor surgery.

We are looking for a competent Health Care Assistant/Phlebotomist who can work independantly and also as part of a team. The successful applicant should have had prevous experience of working in a busy GP surgery carrying out a variety of duties to meet the needs of the practice patients.


Main duties of the job

To provide a high-quality care service based on theneeds of the Practice population which includes treatment, screening,preventative care, and patient education. Assisting GPs/PN/NA

To manage patients with chronic disease, running specificclinic sessions and supporting the maintenance of robust call and recallsystems

* Venepuncture and other skill-based tasks
* Home Visits to Housebound Patients to Manage Long TermConditions
* Assist with Minor Surgery Aseptic Technique
* Looking at the Unmet Needs of all the patients with LTCincluding Housebound
* NHS Health Checks
* Function as chaperone when required
* Undertake health promotion to help educate patients
* Weighing & Measuring Patients
* Undertake Opportunistic Screening
* Update Care Plans and inform all people involved in thepatients care
* Health Promotion Provide individual advice on SmokingCessation, Health Eating & Refer to in-house staff H&WBC, SocialPrescriber
* Develop & Maintain Health Promotion Material with thePractice/Notice Boards
* Refer to other agencies as appropriate
* Infection Control Apply Infection Control Measuresaccording to local and national guidelines. Support patients and colleaguesin adopting sound infection control measures.
* Safeguarding Adults/Children
* Competent in Anaphylaxis and Resuscitation Techniques
* Assisting with Flu Clinics
* 24 Hours ECGs after completing required training

Job responsibilities

* To maintain accurate patient records and enter onto thecomputer using agreed Snomed Codes
* Ensure accurate completion of all necessary documentationassociated with patient health care
* To collect data and participate in regular Practice auditand research projects
* To contribute to the preparation of any Practicedevelopment plans
* Mental Health Recognises and is aware of appropriatereferral mechanisms for family violence, substance a use and addictivebehaviours
* General
* To maintain a safe and therapeutic environment for all staff,patients, carers, and co-workers from other agencies. Ensure all statutoryregulations (e.g. Health & Safety, COSHH, etc) are implemented andadhered to
* To always promote a professional approach by attire andattitude to patients and staff both in and out of the Practice
* To maintain own continuous professional development andensure PDP kept up to date in line with the requirements of the Practice
* Attend and participate in staff and Practice meetings andin-house clinical training sessions
* Any other reasonable duties that may be required from time-to-time


Person Specification


Experience

* Experience of working within a Primary Care Nursing Team
* Experience of dealing with Vulnerable Patients
* Experience of working in a GP Practice


Qualifications

* HCA & Phlebotomy Qualification or equivalent
* Training in chronic disease management
* Educated to GCSE Level or above
* Experience of dealing with Long Term Conditions
* HCA working in Primary Care
* Time Management
