Job overview
The Cancer Support Worker will form an integral part of the multidisciplinary team caring for the holistic needs of patients and their carers, under the guidance of the wider team. The role involves coordinating services, communication between caregivers, treatment providers, and patients, creating a seamless care experience.
The main purpose is to support patients who can self-manage and help them navigate the complex health and social care system during and after cancer treatment (Patient Initiated Follow Up - PIFU).
The post holder will work within a network of Cancer Support Workers at HHFT but will operate independently within the role's scope, under the guidance of the Clinical Nurse Specialist in the breast tumour group.
Main duties of the job
* Assist with coordination and management of patients undergoing treatment
* Support the breast clinical nurse specialists in developing nurse-led services
* Provide information to patients regarding treatment and follow-up, signposting to healthcare providers
* Support development of the PIFU pathway and electronic systems (e.g., MMR)
* Provide telephone advice and triage within protocols, documenting outcomes
* Offer first-line psychological support following appropriate training
* Assist patients in completing Holistic Needs Assessments (HNA)
* Participate in surveys and audits
* Help develop and deliver health and wellbeing sessions (individual or group), focusing on health promotion, disease prevention, and self-management
* Accurately document all interactions in line with Trust policies
* Demonstrate excellent communication skills

Training and Qualifications
* NVQ Level 3 or similar
* GCSE English and Mathematics or equivalent
* Experience with IT systems (Excel, Word)
