Role Purpose

As an Assertive Outreach Nurse, you will work directly with service users currently using substances across Rhyl. The main aim of the role is to engage with these individuals, identify barriers and work with them towards recovery and social stability. The role will be responsible for the delivery of community-based wellbeing and harm reduction substance misuse support services. You will work with a peer mentor team and project partners within the Salvation Army and wider substance use treatment system to support individuals with complex mental, physical and social issues.

Key Duties and Responsibilities

· Provide advice/information and guidance on the management of substance misuse and mental health.

· Undertake comprehensive clinical mental health assessments.

· Carry a caseload of substance using individuals who may / may not be currently engaged in statutory and other voluntary treatment services.

· Agree engagement plans with service users, and review them regularly.

· Ensure case notes and other statutory records are up to date and kept in accordance with Kaleidoscope policies and procedures/regional information sharing protocols.

· Provide advice, support and training to GPs, pharmacists and other professionals where appropriate.

· Participate in clinical audits, service reviews and the evaluation of service user satisfaction, implementing resulting recommendations, in order to improve and develop the service.

· Facilitate the transfer of service users between secondary and primary care.

· Work with other regional treatment services to support the development of the local treatment system, and in particular community mental health services and substance use treatment.

· Actively encourage service user involvement and feedback and ensure views are considered in informing the development of the service.

· Collate data and contribute to service reports.

· Ensure that data recording and reporting requirements are fully met.

· Develop effective links with mental health, GP prescribing and other voluntary and statutory sector providers and agencies.

· To establish individual need, discussing all treatment options (including pharmacological treatment and in-patient assessment where indicated) and potential side effect and overdose risks with each service user.

· To attend and report to safeguarding reviews, meetings and conferences

· To work within agreed policies, procedures and protocols to safeguard children and vulnerable adults.

· To undertake mental health assessments, assess need and agree and review treatment for service users who have a complex care.

· To work collaboratively and proactively with colleagues, to ensure that services are fully coordinated and are working collectively towards the achievement of recovery goals and positive outcomes for everyone.

· Ensure that procedures are constantly reviewed to promote best practice and evidenced based practice in accordance with UK Guidelines on Clinical Management, NICE guidance, and clinical protocols and procedures.

· Participate in clinical audits, service reviews and the evaluation of service user satisfaction, implementing resulting recommendations, in order to improve and develop the service

· To maintain an up-to-date knowledge of medicine management and both local and national guidelines and agreements.

· To attend meetings appropriate to the development and maintenance of the role

· To adhere to Patient Group Directives and ensure requirements on updating are maintained

· To receive regular clinical supervision and maintain a current knowledge of appropriate interventions about substance misuse service users .

· To ensure individuals can access substitute prescribing, sexual health interventions, contraception, vaccination, oral health, wound care and dietary support.

· Deliver harm reduction initiatives throughout the Rhyl community, including Naloxone training and distribution, Outreach Needle Exchange, and Dried Blood Spot Testing.

· Undertake risk assessment and develop and review risk managements plans that seek to mitigate identified risk.

In consultation with the post holder, this job description is liable to variation to reflect actual, contemplated or approved changes in/or to the job role. This role does require the post holder to work outside of normal working hours.

Job Types: Full-time, Fixed term contract

Contract length: 12 months

Pay: £36,250.00 per year

Ability to commute/relocate:

* Rhyl LL18: reliably commute or plan to relocate before starting work (preferred)

Experience:

* mental health/substance misuse : 1 year (preferred)

Licence/Certification:

* NMC (required)
* Driving Licence (required)
