We are recruiting social workers with empathy and the drive to make a difference, finding creative ways to support people to achieve what they want to and live as independently as possible in a place they call their home.
We need 2 permanent, Full-time, Social Workers to work across North Lancashire as part of our Adult Services Older Adults and Physical Disability Community Teams.
This is a hugely exciting time for us. Weve launched Living Better Lives in Lancashire – our new strength-based vision for social care.
Were driving forward innovative practice that will make a difference to our communities and we need new social workers to help us achieve our vision of supporting people to achieve what they want to, living independently in a place they call their home.
As part of your exciting new role, you will be expected to undertake strength-based, outcome-focused assessments and support planning, ensuring that the residents of Lancashire receive the right amount of support.
You will play a vital role in supporting people to access local services, maintain independence, promote health and wellbeing and to live independently.
A key part of the role includes working alongside other agencies, professionals, and informal carers, providing a holistic service.
