We are working closely alongside a Local Authority in Nottingham to assist with the appointment of a Social Worker, on a 2-month contract, likely to be extended at clients discretion. Rate of Pay: £18.16 - £25.01 per hour Summary: The Social Worker (Band B) is responsible for assessing individual needs and coordinating outcome-based plans that promote independence, well-being, and personal choice. Responsibilities: Manage a caseload of complex cases, including assessment, support planning, and reviews. Carry out holistic assessments using health and social care tools, including CHC assessments. Promote and facilitate access to Reablement and enablement services. Apply a strength-based approach to support independence and delay long-term care. Identify and mobilize community and personal support networks and resources. Qualifications: Qualified Social Worker, registered with Social Work England. Evidence of CPD and relevant post-qualifying study. Desirable: Additional qualifications (e.g., BSL, blind/deaf communication training). Essentials: Knowledge of adult community care legislation, policy, and services. Understanding of current adult social care and health policy drivers. Minimum two years post-qualification experience in complex and high-risk settings. Experienced in safeguarding, multi-agency work, and report writing. Skilled in independent workload management and prioritization.
