What the Job Involves As a Social Worker within the Hospital Integrated Discharge Team, you will facilitate safe discharge of service users from hospitals, mental health units, and rehab units. You will work collaboratively with health colleagues to maintain the dignity, choice, and independence of service users and their carers. Your role includes providing advice, information, and signposting to appropriate provision in the voluntary and independent sectors. Working within a hybrid model, you will have both office and home-based workdays, with mandatory office attendance at least one day per week.
