The post holder is a Clinical Pharmacist, who acts within theirprofessional boundaries, supporting and working alongside a team ofpharmacists in general practice.
The post holder will work as part of a multi-disciplinary team in both a remote & patient-facing role.
The post holder will take responsibility for areas of chronic diseasemanagement within the practice and undertake clinical medication reviews toproactively manage patients with complex polypharmacy.
The post holder will provide primary support to general practice staffwith regards to prescription and medication queries. They will help supportthe repeat prescription system, deal with acute prescription requests, andmedicines reconciliation on transfer of care and systems for saferprescribing, providing expertise in clinical medicines advice whileaddressing both public and social care needs of patient in the GP practice.
The post holder will provide clinical leadership on medicinesoptimisation and quality improvement and manage some aspects of the qualityand outcomes framework and enhanced services.
The post holder will ensure that the practice integrates withcommunity and hospital pharmacy to help utilise skill mix, improve patientoutcomes, ensure better access to healthcare and help manage workload.
The role is pivotal to improving the quality of care and operationalefficiencies so requires motivation and passion to deliver excellent servicewithin general practice.

Conduct surgery and telephone consultations with patients with minor illnesses
To plan and organise own workload, including audit and project work, and training sessions for members of the clinical pharmacy team, GP practice team
To maintain registration as a pharmacist and comply with appropriate professional codes.
As appropriate to the post, to maintain and develop professional competence and expertise, keep up to date with medical/therapeutic evidence and opinion, and local and national service, legislation and policy developments, agree objectives and a personal development plan and participate in the appraisal process.
To attend local, regional and national meetings of relevance as agreed with the post holders line manager.
Medication reviews of patients with polypharmacy especially for older people and people with multiple co-morbidities.
Provide advice and support for patients with self-limiting conditions
Reconcile medicines following patients discharge from hospital or clinic appointments
Review medications for newly registering patients
Practice as an independent prescriber working within scope of practice
To undertake any other duties commensurate with the post holders grade as agreed with the post holders line manager.

Job responsibilities
Long-termcondition clinics:
See(where appropriate) patients with single or multiple medical problems wheremedicine optimisation is required (e.g. COPD, asthma).
Reviewthe on-going need for each medicine, a review of monitoring needs and anopportunity to support patients with their medicines taking ensuring they getthe best use of their medicines (i.e. medicines optimisation). Make appropriaterecommendations to GPs for medicine improvement.
Undertakeclinical medication reviews with patients and produce recommendations forsenior clinical pharmacist, nurses and/or GP on prescribing and monitoring.
Carehome medication reviews:
Undertakeclinical medication reviews with patients and produce recommendations for thesenior clinical pharmacist, nurses or GPs on prescribing and monitoring.
Workwith care home staff to improve safety of medicines ordering andadministration.
Domiciliaryclinical medication review:

Answersrelevant medicine-related enquiries from GPs, other practice staff, otherhealthcare teams (e.g. community pharmacy) and patients with queries aboutmedicines.

Providingfollow up for patients to monitor the effect of any changes
Unplannedhospital admissions:
Reviewthe use of medicines most commonly associated with unplanned hospitaladmissions and readmissions through audit and individual patient reviews.
Putin place changes to reduce the prescribing of these medicines to highriskpatient groups.
Managementof medicines at discharge from hospital:
Toreconcile medicines following discharge from hospitals, intermediate care andinto care homes, including identifying and rectifying unexplained changes andworking with patients and community pharmacists to ensure patients receive themedicines they need post discharge.
Setup and manage systems to ensure continuity of medicines supply to highriskgroups of patients (e.g. those with medicine compliance aids or those in carehomes).
Signposting:
Ensurethat patients are referred to the appropriate healthcare professional for theappropriate level of care within an appropriate period of time e.g. pathologyresults, common/minor ailments, acute conditions, long term condition reviewsetc.
Repeatprescribing:
Produceand implement a practice repeat prescribing policy.
Managethe repeat prescribing reauthorisation process by reviewing patient requestsfor repeat prescriptions and reviewing medicines reaching review dates andflagging up those needing a review.
Ensurepatients have appropriate monitoring tests in place when required.
Riskstratification:
Identificationof cohorts of patients at high risk of harm from medicines through pre-preparedpractice computer searches
Medicinesquality improvement:
Undertakeclinical audits of prescribing in areas directed by the practice (to completeall the PQSS and QOF prescribing audits each year and it will be under practicemanagers direction), feedback the results and implement changes in conjunctionwith the practice team.
Implementchanges to medicines that result from MHRA alerts, product withdrawal and otherlocal and national guidance.
Implementationof local and national guidelines and formulary recommendations:
Monitorpractice prescribing against the local health economys RAG list and makerecommendations to GPs for medicines that should be prescribed by hospitaldoctors (red drugs) or subject to shared care (amber drugs).
Assistpractices in seeing and maintaining a practice formulary that is hosted on thepractices computer system.

Person Specification
Experience
* In depth therapeutic and clinical knowledge and understanding of the principles of evidence based healthcare.
* An appreciation of the nature of GPs and general practices.
* An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing.
* Good IT literacy and proficiency with the Microsoft 365 suite.
* Excellent interpersonal, influencing and negotiating skills

* Able to demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences.

Qualifications
* Masters degree in pharmacy (MPharm) or equivalent
