Job Overview
We are seeking an enthusiastic and motivated Pharmacy Technician to join our dynamic team. This is an excellent opportunity to contribute to the delivery of high‑quality medicines optimisation, support chronic disease management, and help improve patient care across the PCN.
Victoria Primary Care Network (VPCN) provides care for approximately 50,000 patients across Victoria Medical Practice, Downlands Medical Centre and Vita Healthcare Practices.
Responsibilities
* Support pharmacists in the delivery of clinical and administrative pharmacy services.
* Assist with medicines optimisation and long term condition management.
* Conduct structured medication reviews.
* Optimise medication based on hospital discharge letters.
* Answer medication and prescription queries from patients and other healthcare professionals.
* Manage repeat prescribing.
* Support the prescription team in day‑to‑day tasks.
* Support implementation of national policies and guidelines.
* Contribute to reducing medication wastage.
* Participate in face‑to‑face clinics in competent clinical fields (full training provided).
* Receive one‑to‑one clinical supervision and support from clinical pharmacists.
* Take part in ongoing training and career progression opportunities.
* Help reduce GP workload through effective pharmacy interventions.
* Ensure compliance with relevant standards, including CQC, NICE, MHRA, QOF, and PCN DES.
* Assist with patient education and adherence support for long‑term conditions.
* Contribute to audits, quality improvement projects, and service evaluations.
* Assist with clinical coding and data collection.
* Participate in multi‑disciplinary team meetings and collaborative care planning.
* Support medicines reconciliation in patients transitioning between care settings.
