Overview

Under the direction of the Urgent Community Response Nurse in charge you must assess, plan, implement and evaluate patient care demonstrating evidence based / best practice, considering your own clinical confidence and competence. You must implement enhanced programs of care. You may be part of the Urgent Community Response Team to provide high standards of clinical intervention and nursing care for service users accessing the range of services, including acting as nurse co-ordinator as required.

To implement enhanced clinical care programs of the highest quality and standards and in line with the Walsall Healthcare NHS Trust policies and procedures. Work within a multi-agency / multi-disciplinary team adopting a ‘no blame’ culture. Adhere to Walsall Healthcare NHS Trust policies, procedures and protocols to promote the quality of patient experience.

To be responsible for the assessment of care needs and the development, implementation and evaluation of programs of care, within Scope of Professional Practice.

Demonstrate high level of judgmental/analytical skills when interpreting and performing patient assessments and care planning. Maintain accurate and up to date nursing records, maintaining patient confidentiality, and statistical records as required by the service. Work with other professionals to promote multidisciplinary working. Develop clinical skills and access training as required / directed to enable participation in future service developments.

Assist in identifying the physical resource needs of the team and contribute to plans for improvements and maintenance of equipment in respect of available resources. Assist in the supervision of qualified and unqualified staff and facilitate their continuing professional development. Ensure effective learning experiences and opportunities to achieve learning outcomes for students and the provision of learning opportunities. To prepare lesson plans, deliver training programs to the multi-disciplinary team members, and to review and elevate these programs. Communicate information to patients and relatives with empathy and reassurance.

Walsall Healthcare NHS Trust is an integrated Trust and the only provider of NHS acute care in Walsall, serving a population of 284,300. It provides inpatient and outpatient services at the Manor Hospital as well as a wide range of services in the community. Walsall Manor houses the full range of district general hospital services under one roof. The £170 million development was completed in 2010, and ongoing upgrades ensure the Trust now has state-of-the-art facilities, including a Critical Care Unit, Neonatal Unit, Obstetric Theatre, and Integrated Assessment Unit. A new Urgent and Emergency Care Centre, opened in March 2023, has significantly improved emergency care facilities and capacity, providing almost 5,000 square metres of additional clinical space.

Key Responsibilities

Care Delivery

* To be responsible for the assessment of care needs and the development, implementation and evaluation of programs of care, within Scope of Professional Practice.

Management and Leadership

* Organise workload, showing appropriate time management in agreement with clinical supervisor.
* Provide excellent evidence based clinical practice.
* Identify poor professional practice and take appropriate action to address it.
* Act in a professional manner at all times whilst on Walsall Healthcare NHS Trusts business and demonstrate excellence in clinical practice, ensuring compliance to Walsall Healthcare NHS Trust policies and procedures.
* Assist the Advanced Care Practitioners in strategies that promote quality assurance, Clinical Governance and quality audit.
* Develop clinical skills and access training as required / directed to enable participation in future service developments.

Support for PPI

* Communicate information to patients and relatives with empathy and reassurance.
* To ensure that patients and users of the service are involved and to assist in providing links with the public and PALS, i.e. to pick up on the themes of complaints and report clinical concerns.
* To participate in service user forums.
* To maintain a patient led focus in all aspects of care.
