The primary care team undertake annual physical checks for patients on the Serious Mental Illness register within primary care.
This Healthcare Support Worker is an integral part of this small team, working to ensure that all SMI patients are receiving a health check with appropriate signposting where needed.
The post holder is supported by a Registered Nurse
The post holder must hold a full, valid, UK driving licence and have access to a car/vehicle to use for business purposes (unless you have a disability as defined by the Equality Act 2010).
Main duties of the job
* The Healthcare Worker is an integral part of the small Primary Care Physical Health Team, undertaking and recording annual Physical Health Checks for patients on the Serious Mental Illness register.
* Deliver an effective and competent level of care under the guidance andsupervision of a Registered Nurse.
* Monitor, report, and maintain a healthy, safe and secure workplace.
* Consistently delivering a patient-focused service.
* Provide and receive routine information orally, in writing or electronically to inform others as appropriate
* Offer appropriate support and advice to patients.
* Be able to adapt communication methods according to the needs of the individual.

Job responsibilities
* Enable patients to maintain and improve their independence by promoting health and wellbeing.
* Undertake physical health checks and physical health care as directed by the Registered Nurse and service specification, reporting outcomes on the Primary Care electronic system.
* Report physical health concerns identified during the checks to the GP or practice nurse.

Person Specification
* Health and Social Care Qualification
* Care certificate completed/willingness to work towards
Experience
* Experience of undertaking basic health checks
* Phlebotomy training
* Use of ECG and other medical devices
knowledge and skills
* Able to communicate effectively to a wide range of people
* Able to record interventions in a timely manner
* Able to undertake physical health checks and document on EMIS
* Numeracy and English literacy skills
