Clinical Pharmacist
GP Surgery - Todmorden, West Yorkshire
Salary: Up to £60,000 per annum
Hours: Part-Time or Full-Time (can work 37.5 hours over 4 days)

Job Summary
A GP Surgery in Todmorden, West Yorkshire, comprising two practices with around 35,000 patients, is seeking an experienced Clinical Pharmacist to join its established pharmacy team.
You will work closely with GPs, pharmacy colleagues, and the wider multidisciplinary team to deliver safe, effective, and patient-centred care. This role is ideal for a pharmacist with strong clinical skills and a collaborative approach to medicines optimisation.
Main Duties
Conduct structured medication reviews (SMRs) for patients with complex polypharmacy
Support repeat prescribing, medicines reconciliation, and post-discharge care
Provide clinical medicines expertise to patients and general practice teams
Improve prescribing quality through audits, service reviews, and quality improvement initiatives
Participate in pharmacist-led clinics, including hypertension services
Support delivery of enhanced services
Work collaboratively with GPs, care homes, community pharmacy, secondary care, and ICB pharmacy teamsPerson Specification (Summary)
Essential
MPharm degree and GPhC registration
Minimum of 2 years' post-qualification experience
Experience in patient-facing medication reviews and working with GPs
Strong communication, organisational, and clinical decision-making skills
Knowledge of primary care prescribing and medicines optimisation
Commitment to reducing health inequalities
Flexibility to work across surgery sites and full UK driving licenceDesirable
Independent Prescriber qualification (or working towards)
Experience in General Practice or GP surgery settings
Knowledge of GP clinical systems (EMIS, SystmOne)
