Overview
Hospital at home (H@H) is a short-term, targeted intervention that provides a level of acute hospital care in an individual's own home environment that is equivalent to that provided within a hospital. This includes use of hospital level diagnostic tools such as bladder scanners, ECG and offers hospital level treatment such as Intravenous medications, oxygen and fluids.
The key aims of H@H are prevention of admission to hospital and facilitating early supported discharge from hospital with the focus on patient choice and personal outcomes.
Responsibilities
* Deliver a high standard of safe and effective medical care within patients\' homes; work autonomously and recognise and respond to deterioration.
* Utilise high level clinical skills including venepuncture, ECG, IV additives and cannulation.
* Maintain excellent communication and time management skills.
Qualifications
* Valid NMC registration and a flexible attitude to work with the ability to travel within the area in a timely manner is essential.
* Flexibility to work over a 7-day period, with shifts from 08:00 to 22:00.
* Continuous Professional Development is supported; staff are encouraged to attend regular supervision and annual appraisals.
* PVG Scheme membership is required prior to appointment.
