We are looking for a Practice Nurse to join ourteam. This is a part-time (20-30 hours per week) permanent role.
Main duties of the job
Desirable skills and interests would be in chronicdisease management in particular respiratory management, immunisations, woundcare, and health screening and promotion.
About us
We are a supportive practice with a diverse patientpopulation. We have an inner-city location and are actively involved with the PrimaryCare Network, local provider company (federation), ICB, medical school and GP Training Programme.
The practice operates the Year of Care model for chronic disease management and holds regular nurse meetings and educational sessions.

Qualifications
* Registered General Nurse with evidence of current NMC registration
* Diplomas in chronic disease management e.g. Respiratory, Diabetes, CHD and willingness to work towards any not yet obtained
* Qualifications/signed competencies in the activities listed below and a willingness to work towards any not yet obtained
