Job Overview
We are looking to recruit a motivated and enthusiastic Mental Health Practitioner to join the Mental Health Intensive Support Team (MhIST), an award winning and forward-thinking intensive community mental health rehabilitation service. The post will be based in Macclesfield. MhIST is a recovery-orientated service offering specialist assessment, intervention and support to individuals with complex mental health needs across Cheshire & Wirral. The multidisciplinary team works with patients and their families and social networks to support meaningful rehabilitation goals and recovery.
We value professional development and provide opportunities for ongoing learning, including quarterly team training days and protected reflective spaces. We welcome applicants from Nursing, Occupational Therapy and Social Work.
Main duties of the job
* Work intensively with a small cohort of patients experiencing a range of complex mental health needs, providing high-value, recovery-focused care.
* Proactively prevent crises and support patients to remain in the community; demonstrate strong risk management, adaptive care planning and communication skills.
* Collaborate with a multidisciplinary team to deliver holistic, need-adapted community rehabilitation.

Qualifications
Essential criteria
* Educated to diploma/degree level and registered with the appropriate professional body
* Professional knowledge through a relevant degree and experience with complex cases
Desirable criteria
* Evidence of continued professional development
* NVQ A1 qualification
* Relevant mentorship qualification as required by governing body
* Specialist practitioner qualification (SPQ)
Knowledge and Expertise
Essential criteria
* Recovery-focused philosophy knowledge and experience
* Experience delivering individual service user-focused assessments
* Experience with evidence-based, measurable interventions guiding recovery
* Knowledge of psychosocial interventions (PSI)
* Understanding of physical health needs
* Community-based practice experience
* Ability to develop care programs and packages
* Experience in recording personal information and working with commissioners and care providers
Desirable criteria
* Knowledge of Mental Health Act and Mental Capacity Act
* Experience with surveys/audits, research and development, or clinical trials
* Knowledge of complex care management and bespoke support packages
Experience
Essential criteria
* Minimum of 2 years post-registration experience working with adults with complex mental health problems
* Experience planning and organizing complex activities or programmes requiring formulation and adjustment
* Experience developing specialist care programs and packages
* Experience providing specialist care advice
* Experience collaborating with commissioners and care providers
Desirable criteria
* Experience contributing to policy improvements, safe management of equipment/resources, or professional supervision
* Experience of audits, R&D, or surveys; equipment testing/adaptation; rehabilitation framework
Employer certification / accreditation
