We are excited to announce an opening for a Frailty Pharmacist (Band 7) in our newly established Frailty Unit at NHS Borders. This is a permanent 1.86 WTE position within the Pharmacy Department at Borders General Hospital.
Key Responsibilities
* Conduct comprehensive medication reviews for frail patients.
* Collaborate closely with healthcare professionals to develop individualised care plans.
* Provide expert pharmaceutical advice to enhance patient safety and medication efficacy.
* Educate patients and caregivers about their medications and health conditions.
